Student Health Packet: Immunization & Health Forms

In the interest of maintaining a safe and healthy campus, all full and part-time students, new to NJIT, are required to complete the Pre-Entrance Health Requirements packet.

This includes all new first time students (full and part-time, international, undergraduate, graduate and doctoral).

Student Health Packet

All forms in the Student Health Packet are to be submitted through the Medicat Patient Portal at njit.medicatconnect.com.
